Infusion set material

Disposable infusion sets are commonly used consumables in intravenous infusion therapy, and their main materials can be divided into polyvinyl chloride (PVC), polyolefin thermoplastic elastomer (TPE), ultra-low density polyethylene (PE) and polypropylene (PP), and so on.

Since the disposable infusion set is in direct contact with the drug, the adsorption of the drug by its material has increasingly become a focus of attention. Among them, PVC infusion sets have been widely controversial because of the plasticizer dioctyl phthalate (DEHP). DEHP can accumulate in tissues, blood, and body fluids and cause liver toxicity and reproductive toxicity. DEHP is an endogenous interference. Factors also exist carcinogens.

Clinical nurses need to understand the characteristics of disposable infusion set materials, as well as commonly used drugs that are easily absorbed by different materials, which is conducive to ensuring the effectiveness and safety of the clinical medication. Filter function

The American "Standard of Nursing Practice for Infusion Therapy" has a higher standard recommendation for parenteral nutrition infusion: "When entering parenteral nutrition without fat emulsion, a 0.2μm filter should be used. Entering parenteral nutrition containing fat emulsion (3-in-1 formulation), a 1.2μm filter should be used to reduce the risk of microbial, sediment, or particle contamination.

The particles in the infusion refer to the small particles of impurities that are polluted by various means during the production or clinical use of the medicinal solution, and the diameter is mainly between 1-25μm. Infusion of microparticles can cause phlebitis, vascular embolism, granuloma formation, and pyrogen-like reactions.

At present, ordinary disposable infusion sets limit the terminal filter infusion set to 15-20μm, which cannot achieve the purpose of intercepting harmful particles in the infusion solution. The pore size of the filter membrane of the precision filtration infusion device is less than or equal to 5μm.

When the medicinal liquid reaches the precision filtration device, the insoluble particles are intercepted by the precision filtration device, thereby effectively preventing the insoluble particles in the medicinal solution from entering the blood and reducing the potential harm or damage caused by the particles to the patient. direct harm, so as to achieve the purpose of treatment.

Precision filtration infusion sets are more expensive than ordinary infusion sets and are mostly made of non-PVC materials. They are suitable for most drugs and ensure the safety of infusion, and are widely used in clinical practice.

If the infusion of the precision filter infusion set is not smooth during use, it should be checked whether the side hole of the infusion set is blocked by rubber particles and whether the input liquid volume exceeds 2,000 ml. When the particles in the filter disc exceed the saturation volume, the filter hole will be completely closed. If blocked, the infusion set should be replaced.

Avoid air in the filter disc. When the infusion tube is used for the first time, the air should be exhausted. Observe frequently to avoid emptying of the medicine.

Light-proof function

During the infusion process, due to the different properties of the drugs, there are different requirements in the use process. Some drugs are easily decomposed when exposed to light, which reduces the efficacy of the drugs, such as some antitumor drugs, antibiotics, antihypertensive drugs, and vasodilators. Drugs, water-soluble vitamins, etc., so needs to be protected from light during the infusion process.

The pipeline and a dripping pot of the disposable light-proof infusion set adopt a two-color molding process. If the pore size is too small, it is easy to be blocked by micro polymers of platelets and leukocytes in blood cells, which affects the effective filtering area. In view of the PVC material of the blood transfusion set and the large filter aperture, it is not suitable for infusion.
